The short answer

The biggest longevity medicine news of August 2026 was regulatory: the UK authorized Eli Lilly’s oral GLP-1 orforglipron (Foundayo), and the FDA added cardiovascular-risk reduction to Mounjaro (tirzepatide) labeling after the 13,299-person SURPASS-CVOT trial. Dr. Dusan Sajic’s August Trend Radar for Sajic Longevity covers those plus five more approvals and clearances and three peer-reviewed studies, each graded by the kind of evidence behind it.

  • GLP-1 care moved on two fronts: a needle-free tablet in the UK, and a heart-outcomes label for tirzepatide in type 2 diabetes.
  • Aesthetics gained anatomy-specific tools: knee laxity (Ultherapy PRIME), under-eye hollowing (RHA Redensity Eye) and chin structure (Restylane Shaype, EU).
  • Measurement got sharper: a new skin-sagging metric, a ranking of which epigenetic clocks respond to interventions, and FDA-cleared sarcopenia-risk screening.
  • A supercentenarian study points to an unusual immune adaptation around age 100, a research clue rather than a treatment.
  • None of these replace individual assessment: eligibility, safety screening and measurable follow-up still decide whether an option fits you.

August’s radar is led by seven meaningful regulatory milestones. In parallel, peer-reviewed work sharpened how clinicians may measure skin aging, biological aging, and exceptional immune resilience. Together, these developments reinforce a patient-first model that pairs innovation with careful selection, safety screening, and measurable outcomes. Stories are ranked by engagement, shareability and clinical relevance.

What happened in longevity medicine in August 2026?

Ten developments mattered most, and they are not all the same strength of evidence. A regulatory approval built on a randomized outcomes trial is a different thing from a company-reported satisfaction figure or a single-cell observation in a handful of exceptional people. The table grades each one by the type of evidence behind it, so you can weigh the headline accordingly.

Evidence grades are an editorial classification of the type of evidence described in each announcement or paper, not a formal GRADE rating.

01 · GLP-1

1. Is there an oral GLP-1 pill approved in the UK? Orforglipron (Foundayo)

Yes. The UK became the first country in Europe to authorize the oral GLP-1 orforglipron: a once-daily, needle-free tablet that cleared its first European regulatory hurdle for both weight management and type 2 diabetes.

What it is. The UK Medicines and Healthcare products Regulatory Agency (MHRA) authorized Eli Lilly’s orforglipron, branded Foundayo, on August 10 for weight loss and weight maintenance alongside diet and activity. The authorization covers adults with a BMI of at least 30, or a BMI of 27 to under 30 with at least one weight-related comorbidity. The tablet can be taken once daily without food or water restrictions. It is also authorized to improve glycaemic control in insufficiently controlled type 2 diabetes.

Why it matters for patients. A non-injectable option could broaden access and adherence, but eligibility, adverse effects, body-composition monitoring and long-term maintenance still require individualized clinician oversight. The next question is not simply access; it is how to pair metabolic treatment with nutrition, resistance training and durable follow-up. That is how we run our GLP-1 weight loss program, and why protein and lean mass come first in our guide to protecting muscle on a GLP-1.

Evidence grade: regulatory authorization.

2. Does tirzepatide reduce heart attack and stroke risk? The new Mounjaro label

For high-risk adults with type 2 diabetes, the FDA now says yes: tirzepatide is approved to lower major cardiovascular-event risk in that group.

What it is. On August 28, the FDA approved Mounjaro (tirzepatide) to lower the risk of cardiovascular death, non-fatal myocardial infarction, or non-fatal stroke in adults with type 2 diabetes at high risk for these events. The decision follows SURPASS-CVOT, which randomized 13,299 participants across 30 countries. Tirzepatide was non-inferior to dulaglutide, with an estimated MACE-3 hazard ratio of 0.92 (95.3% CI, 0.83 to 1.01). Superiority versus dulaglutide was not established.

Why it matters for patients. For appropriately selected patients, cardiometabolic goals can now be discussed in one treatment framework, while clinicians continue to screen contraindications and manage gastrointestinal and other safety risks. It is a reminder that obesity and diabetes care should be about more than a number on the scale.

Evidence grade: regulatory approval backed by a large randomized cardiovascular outcomes trial. Note the comparator: the trial showed non-inferiority to another active drug, not superiority.

3. Can you tighten loose skin on the knees without surgery? Ultherapy PRIME

There is now an FDA-cleared option: Ultherapy PRIME is the first energy-based device cleared for the appearance of knee skin laxity, moving non-surgical body tightening beyond the usual face-and-neck territory.

What it is. Merz Aesthetics announced FDA clearance on August 11 for Ultherapy PRIME to improve the appearance of skin laxity on the knees. The microfocused-ultrasound platform with real-time visualization is now indicated across the face, décolleté, abdomen, arms and knees. Merz reports more than 4 million Ultherapy treatments globally and cites 95% satisfaction at one year for the platform’s legacy experience.

Why it matters for patients. Patients concerned about localized knee crepiness have a new non-surgical, energy-based option, but candidacy should account for laxity severity, skin quality, expected downtime and realistic remodeling timelines. Body aging is not confined to the jawline.

Evidence grade: FDA clearance; treatment volume and satisfaction figures are company-reported.

4. Is there an FDA-approved filler for under-eye hollows? RHA Redensity Eye

Yes. A purpose-designed hyaluronic-acid filler, RHA Redensity Eye, now has an FDA indication for infraorbital hollowing, one of facial aesthetics’ most technically demanding zones.

What it is. Revance and Teoxane announced FDA approval on August 20 for RHA Redensity Eye to correct moderate-to-severe infraorbital tissue-volume deficiency in adults aged 22 and older. In the prospective, randomized, evaluator-blinded 52-week pivotal study, 93% of patients showed aesthetic improvement at three months, with results reported for up to 12 months without touch-ups. The product uses a high dynamic-strength and stretch profile tailored to the expressive under-eye area.

Why it matters for patients. Under-eye hollowing needs anatomy-led assessment and an experienced injector. This approval adds an evidence-backed option but does not eliminate known filler risks, including rare vascular complications. The under-eye is not a one-size-fits-all treatment zone; I go through who is and is not a candidate in the under-eye tear trough filler guide.

Evidence grade: FDA approval; randomized, evaluator-blinded 52-week pivotal study (results as announced by the companies).

5. What is Restylane Shaype? EU approval for non-surgical chin augmentation

Restylane Shaype is a bone-mimicking hyaluronic-acid gel now approved in the European Union for temporary augmentation of the chin, expanding the non-surgical toolkit for structural lower-face shaping.

What it is. Galderma announced EU Medical Device Regulation approval for Restylane Shaype. The new NASHA HD technology is designed for deep injection and structural support. In the cited 12-month pivotal study, 83% of treated people showed improved definition at three months and 66% at 12 months. Eighty-four percent said they would agree to treatment again.

Why it matters for patients. For patients seeking lower-face balance without surgery, this adds another temporary option, but an in-person assessment must distinguish projection needs from age-related volume loss, laxity, or bite-related anatomy. Facial balancing is increasingly about structure, not just volume.

Evidence grade: EU regulatory approval; 12-month pivotal study figures as reported by the manufacturer. Note the drop in responders from three months (83%) to 12 months (66%).

6. How is AI being used in pathology? FDA clears AI slide quality control

AI is moving earlier in the pathology workflow, helping laboratories catch image-quality problems before they slow diagnosis, rather than making the diagnosis itself.

What it is. Leica Biosystems announced multiple FDA 510(k) clearances on August 11, including Aperio iQC DX, described as the first standalone AI-assisted digital-pathology quality-control software cleared for clinical use. It automatically identifies six common slide-image artifacts while slides remain on the scanner. In Leica’s cited real-world study, the AI detected up to 24% more artifacts missed by histotechnicians and reduced hands-on review time by 69%.

Why it matters for patients. This is workflow infrastructure rather than a consumer diagnostic, but faster and more consistent slide quality could support timely dermatopathology and cancer-care decisions when implemented with expert human review. That matters for every skin biopsy.

Evidence grade: FDA 510(k) clearance; performance figures from a company-cited real-world study.

7. Can skin sagging be measured objectively? Skin gravitational stretchability

A new peer-reviewed metric suggests it can be: skin gravitational stretchability (SGS) aims to quantify sagging resilience rather than relying only on visual grading.

What it is. A Scientific Reports paper published August 22 defined SGS using a non-invasive tensile method that simulates gravitational pull on facial skin. Higher SGS correlated with established elasticity and recovery parameters, and a clinical evaluation of a facial cream found reduced visible sagging alongside a statistically significant, time-dependent SGS increase. In vitro and ex vivo experiments linked the response to extracellular-matrix and longevity-marker changes plus protection against reactive oxygen species.

Why it matters for patients. SGS is promising as a research and product-evaluation endpoint, but it remains an emerging metric, not yet a stand-alone clinical test for choosing an anti-aging treatment. It fits a broader shift toward measuring skin quality rather than just photographing it, which I cover in the four parameters of skin quality.

Evidence grade: peer-reviewed study; emerging research metric, early clinical evaluation.

8. Which epigenetic aging clocks actually respond to treatment? The TranslAGE analysis

In the largest harmonized review of human longevity-intervention methylation data, newer measures, especially DunedinPACE and PCGrimAge, were the most responsive biological-age signals.

What it is. Researchers reporting in Nature Medicine curated TranslAGE, a database of 51 public and private longitudinal intervention studies, and calculated 16 prominent epigenetic clocks plus 94 additional DNA-methylation biomarkers. Across the analysis, 19 interventions significantly decreased epigenetic age across the clock set, including 13 after multiple-testing correction. DunedinPACE showed the largest pooled decrease (mean effect size −0.0891; P = 0.0012), while PCGrimAge showed the strongest statistical evidence (P = 0.0003).

Why it matters for patients. Better biomarker selection could make future aging trials more efficient, but a favorable clock change is not yet proof that a treatment will extend lifespan or prevent disease for an individual patient. Longevity care needs better scorecards. We use biological-age testing as one input rather than a verdict; see our diagnostics page.

Evidence grade: peer-reviewed pooled analysis of intervention studies; measures biomarker change, not clinical outcomes.

9. What makes supercentenarians’ immune systems different? Cytotoxic CD4 T cells

Single-cell profiling points to a distinctive immune adaptation, an expansion of cytotoxic CD4 T cells beginning around age 100, that may help explain exceptional healthspan.

What it is. A Cell Reports study published August 19 profiled T cells in supercentenarians and found CD4 cytotoxic T cells expanding around age 100 without signs of exhaustion. The large clones averaged 33.3% among top clones, indicating repeated antigen exposure. Their CDR3β sequences overlapped with T cells expanded in tumors, particularly lung cancer. Ex vivo stimulation also showed diverse cytokine profiles within individual clones.

Why it matters for patients. The result is a compelling clue about immune resilience in exceptional aging, not a current screening test or treatment. It underscores why immune health remains central to longevity research, a thread that also ran through July’s thymus story.

Evidence grade: peer-reviewed observational, single-cell study in a small, exceptional population; mechanistic clue, not yet a clinical intervention.

10. How is sarcopenia risk measured? FDA clears SOZO bioimpedance assessment

A routine bioimpedance scan can now flag sarcopenia risk, bringing muscle measurement closer to everyday oncology and metabolic care.

What it is. ImpediMed announced FDA 510(k) clearance on August 18 for its SOZO Digital Health Platform to assess clinical sarcopenia risk. The platform is described as the only FDA-cleared bioimpedance-spectroscopy solution for this indication and uses 256 frequencies to estimate lean and skeletal-muscle mass. The company notes that roughly one in three breast-cancer patients develops sarcopenia during chemotherapy, and future SOZO v6.3 software is expected to introduce the capability in fall 2026.

Why it matters for patients. Because weight and BMI can conceal muscle loss, routine body-composition surveillance can prompt earlier nutrition and resistance-training intervention, especially during cancer treatment or medication-driven weight loss. Weight alone cannot tell us whether someone is getting stronger or losing muscle.

Evidence grade: FDA 510(k) clearance; figures company-reported, and the capability ships with a future software release.

What do August’s signals mean if you are a patient?

Three practical themes run through the month.

Frequently asked questions

Is oral orforglipron (Foundayo) approved?

In the UK, yes. The MHRA authorized Eli Lilly’s orforglipron, branded Foundayo, on August 10, 2026, for weight loss and weight maintenance in adults with a BMI of at least 30, or 27 to under 30 with a weight-related comorbidity, and for glycaemic control in type 2 diabetes. It is taken once daily without food or water restrictions. Eligibility and monitoring still require a clinician.

Does Mounjaro (tirzepatide) now have a heart-protection indication?

Yes, for a specific group. On August 28, 2026, the FDA approved Mounjaro to lower the risk of cardiovascular death, non-fatal heart attack or non-fatal stroke in adults with type 2 diabetes at high risk for these events, based on the 13,299-person SURPASS-CVOT trial. Tirzepatide was non-inferior to dulaglutide; superiority was not established.

Is there an FDA-approved filler for under-eye hollows?

Yes. RHA Redensity Eye was approved by the FDA on August 20, 2026, for moderate-to-severe infraorbital hollowing in adults 22 and older. In the 52-week pivotal study, 93% showed improvement at three months. Known filler risks, including rare vascular complications, still apply, so anatomy-led assessment and an experienced injector matter.

Can an epigenetic clock prove a longevity treatment works?

Not yet. The TranslAGE analysis in Nature Medicine found that DunedinPACE and PCGrimAge were the most responsive biomarkers across 51 intervention studies, but a favorable clock change is not proof that a treatment will extend lifespan or prevent disease for an individual patient.

What is skin gravitational stretchability?

Skin gravitational stretchability (SGS) is a non-invasive tensile measure, described in Scientific Reports in August 2026, that simulates gravitational pull on facial skin to quantify sagging resilience. It correlated with established elasticity and recovery parameters, but it remains an emerging research metric, not a stand-alone clinical test.

Why does sarcopenia screening matter during GLP-1 treatment?

Weight and BMI can conceal muscle loss. Body-composition surveillance, such as the newly FDA-cleared SOZO bioimpedance sarcopenia-risk assessment, can prompt earlier nutrition and resistance-training intervention during medication-driven weight loss or cancer treatment.
